INDIVIDUALS OF ANY AGE WHO NEED NURSING HOME LEVEL OF CARE MAY HAVE OPTIONS FOR RECEIVING HOME AND COMMUNITY-BASED SERVICES TO AVOID ENTERING A CARE FACILITY. BESIDES MEDICAID SERVICES, SERVICE OPTIONS INCLUDE: CHILD CARE, ADULT DAY HEALTH SERVICES, INDEPENDENCE SKILLS MANAGEMENT, RESPITE CARE, TRANSPORTATION, CHORE, ASSISTED LIVING, ASSISTIVE TECHNOLOGY, HOME MODIFICATIONS, NUTRITION COUNSELING, HOME DELIVERED MEALS AND PERSONAL EMERGENCY RESPONSE SYSTEMS.
