REPORT ALLEGED SOCIAL SECURITY FRAUD. ALSO CAN REPORT FRAUD ONLINE OR TO ANY SOCIAL SECURITY OFFICE. THE FOLLOWING INFORMATION IS HELPFUL TO INCLUDE IN YOUR REPORT: NAME, ADDRESS, TELEPHONE NUMBER AND SOCIAL SECURITY NUMBER OF THE PERSON SUSPECTED OF FRAUD. ALSO INCLUDE PERSON'S DATE AND PLACE OF BIRTH AND FATHER'S AND MOTHER'S NAME IF KNOWN. INCLUDE A COMPLETE DESCRIPTION OF THE POTENTIAL FRAUD INCIDENT AND YOUR NAME, ADDRESS AND TELEPHONE NUMBER. IF AN INDIVIDUAL CANNOT REACH A REPRESENTATIVE ON THE FRAUD HOTLINE BETWEEN 10-4PM EST, CAN REPORT DIRECTLY TO ANY SOCIAL SECURITY OFFICE. THEY WILL TAKE THE INFORMATION AND SEND IT DIRECTLY TO THIS OFFICE.
