HOPE RELIES ON A NETWORK OF VOLUNTEER PHYSICIANS, NURSE PRACTITIONERS, NURSES, DENTISTS, HOSPITALS, HEALTH SYSTEMS, PHARMACEUTICAL MANUFACTURERS, INDIVIDUALS AND OTHERS TO SERVE PATIENTS WHO WOULD OTHERWISE GO WITHOUT CARE. HOPE WORKS THROUGH SEVERAL AGENCIES AS AN ENTRY POINT OR GATEWAY TO SERVICES. SOME PEOPLE REQUIRE TREATMENT BEYOND THE SCOPE OF SERVICES AVAILABLE IN A HEALTH CENTER OR CLINIC. HOPE WORKS AS A VOLUNTEER BROKER FOR PATIENTS COORDINATING CARE FOR LOW-INCOME, UNINSURED PATIENTS WITH 500 SPECIALISTS, AREA HOSPITALS AND HEALTH SYSTEMS AND OTHERS WHO DONATE THEIR TIME AND RELATED CHARGES FOR THE COST OF CARE. TESTS OR PROCEDURES NEEDED FOR DIAGNOSES ARE ALSO DONATED. SAME IS TRUE FOR DENTAL AND VISION CARE. THE PHARMACY AND MEDICATION ASSISTANCE PROGRAM (MAP) PROVIDES SERVICES TO PATIENTS OF SPECIFIC HEALTH CENTERS AND CLINICS AT AN AFFORDABLE COST PLUS A DISPENSING FEE. THE PHARMACY IS BI-LINGUAL AND PROVIDES PATIENT COUNSELING AS WELL AS FILLING PRESCRIPTIONS. FUNDS FROM A NUMBER OF COMMUNITY SOURCES PROVIDE A LIMITED AMOUNT OF SUPPORT FOR THE COST OF MEDICATIONS FOR THE MOST NEEDY IN THE COMMUNITY. ALSO PROVIDES BEHAVIORAL AND MENTAL HEALTH SERVICES.

Eligibility: 1) MUST RESIDE IN THE 3 COUNTY OMAHA METROPOLITAN AREA (DOUGLAS, SARPY OR POTTAWATTAMIE, IA) AND/OR UTILIZE HOPE'S NETWORK CLINICS OR HEALTH CENTERS. 2) MUST NOT BE COVERED BY ANY OTHER KIND OF HEALTH INSURANCE, INCLUDING MEDICAID, KIDS CONNECTION, MEDICARE (EXCEPT FOR MEDICATION NEEDS) VETERANS HEALTH CARE BENEFITS OR DOUGLAS COUNTY GENERAL ASSISTANCE PRIMARY CARE NETWORK. 3) FAMILY INCOME MUST FALL AT OR BELOW 200% OF FEDERAL POVERTY GUIDELINES (APPROX. $41,300 FOR A FAMILY OF FOUR).
