A HEALTHCARE ACCESS PROGRAM SERVING 21 SOUTHEAST NEBRASKA COUNTIES; DIVIDED INTO 6 SEPARATE NETWORKS. A PUBLIC HEALTH NURSE ANSWERS THE PHONE AND ASSISTS THE INDIVIDUALS CALLING IN BY ASSESSING THEIR NEEDS THROUGH A HEALTH ASSESSMENT TOOL AND SCREENED FOR APPROPRIATE PROGRAMS. THE PROGRAMS INCLUDE: CALL CARE EMERGENCY ROOM FOLLOW-UP INFORMATION AND REFERRAL WELL CHILD HEALTH CHECK REMINDER IMMUNIZATION CLINICS LINK WITH COMMUNITY SERVICES (SERVICES AVAILABLE IN THE AREA ARE EXPLAINED) ESTABLISHMENT OF A MEDICAL HOME (MEDICAL, DENTAL AND VISION) EVERY WOMAN MATTERS KIDS CONNECTION MEDICATION ASSISTANCE PROGRAM
