Description
Serves Lincoln and Southeast NE area. Assistance for those with brain injuries and their families to identify and access information, services, and supports to make informed choices and meet or exceed their goals. 1) Intake & Referral-Serves as a point of contact for families and individuals seeking assistance. 2) Case Management-For those needing more than just referrals to other services a personalized plan can be developed that includes setting goals, monitoring progress, and evaluating outcomes. 3) Awareness & Education- Networking with community providers and service systems to educate others about the unique needs of those with brain injury.
