FINANCIAL ASSISTANCE INCLUDING PRESCRIPTION DRUG COINSURANCE, COPAYMENTS, DEDUCTIBLES, HEALTH INSURANCE PREMIUMS AND OTHER SELECTED OUT-OF-POCKET HEALTH CARE COSTS, INCLUDING TRAVEL COSTS.
