HOUSING SERVICES: UNIT FOR MIGRANT FARM LABOR/TRANSITIONAL HOUSING PROJECT IN BAYARD, ALLIANCE & SCOTTSBLUFF; AFFORDABLE DUPLEX UNITS FOR SENIORS IN POTTER; A SIDNEY PROJECT OF 48 APARTMENTS. COMMUNITY HOUSING DEVELOPMENT ORG- WORK TO SECURE HOME MODIFICATIONS THROUGH ATP; REHABILITATION- BRINGS HOMES TO CODE IN MORRILL & SCOTTS BLUFF COUNTIES-HOUSING PRESERVATION GRANT; MODIFIES HOMES FOR PERSONS WITH DISABILITIES USING MAKING HOMES ACCESSIBLE PROGRAM; WEATHERIZATION. YOUTH SERVICES: TRANSITIONAL LIVING-YOUTH AGES 16-21 PREVENTS LONG-TERM DEPENDENCY ON SOCIAL SERVICES. MENTORING. VISITATIONS SERVICES- ENSURE SAFETY OF FOSTER CHILDREN DURING SCHEDULED VISITS WITH RELATIVES; YOUTH SHELTER- PROVIDES EMERGENCY, CRISIS INTERVENTION/SHELTER FOR YOUTH AGES 0 TO 18. FAMILY & COMMUNITY SERVICES: FAMILY DEVELOPMENT PROJECT (CASE MANAGEMENT)- PROVIDES A FAMILY CENTERED PROGRAM EMPHASIZING SELF-SUFFICIENCY & FAMILY STRENGTH. THE PROGRAM UTILIZES EMERGENCY SERVICES, ASSESSMENT, & GOAL SETTING AS WELL AS REFERRALS TO APPROPRIATE AGENCIES. EMERGENCY ASSISTANCE SUCH AS RENT, UTILITY, FOOD, ALSO CAR SEAT RENTAL, INCOME TAX; AMERICORPS; FOSTER GRANDPARENT PROGRAM; DAY CARE CENTERS, HEAD START, & OTHER SITES; HOMELESS FAMILY PROGRAM- EVER GREEN HOUSE. EARLY CHILDHOOD PROGRAMS: HEAD START- CHILDREN AGES 3-5. CENTER BASE & HOME BASE; CHILD DEVELOPMENT CENTER (SCOTTS BLUFF)- A FULL DAY CHILD CARE FOR INFANTS & CHILDREN THROUGH AGE 4; EARLY HEAD START- SERVES FAMILIES, EITHER EXPECTING THEIR FIRST CHILD, OR WITH A CHILD UP TO AGE 3. NE MIGRANT HEAD START- INCOME ELIGIBLE MIGRANT/ SEASONAL FARMWORKER CHILDREN AGES INFANT TO 4 YEARS. HOME-BASED SERVICES SEPT-MAY; PROJECT SUCCESS- PROVIDES FULL CHILD CARE FOR TEEN PARENTS ENROLLED IN GERING & SCOTTSBLUFF. THE PROGRAM INCLUDES CHILD DEVELOPMENT/PARENTING CLASSES, TUTORING, SOCIAL SERVICES COORDINATION, CAREER PLANNING & TRANSPORTATION AS NEEDED. HEALTH & NUTRITION SERVICES: DENTAL CLINIC; PRIMARY HEALTH CARE CLINIC FOR ALL AGE GROUPS, PRENATAL CARE, X-RAY, LABORATORY, PEDIATRIC CARE, GERIATRIC CARE, WELLNESS CARE, & HEALTH EDUCATION; COMMODITY SUPPLEMENTAL FOOD- PROVIDES SUPPLEMENTAL FOOD & NUTRITION INFO TO WOMEN, INFANTS, CHILDREN, & SENIORS; DIABETES CONTROL PROJECT; HIV/AIDS COUNSELING; IMMUNIZATION SERVICES- PROVIDES FREE IMMUNIZATIONS FOR CHILDREN AGES 2-18. VACCINATIONS ARE AVAILABLE FOR ADULTS; LEAD SCREENING PROGRAM; MATERNAL CHILD HEALTH MIGRANT HEALTH PROGRAM- P.H.O.N.E.- KIDS CONNECTION IS AN INSURANCE PROGRAM THAT PROVIDES HEALTH COVERAGE TO UNINSURED CHILDREN ALL ACROSS THE STATE; EVERY WOMAN MATTERS- REPRODUCTIVE HEALTH CARE WIC- PROVIDES NUTRITION EDUCATION & SUPPLEMENTAL FOODS TO WOMEN, INFANTS, & CHILDREN.
