ADMINISTRATIVE OFFICE PURPOSE OF THE PROGRAM IS TO ADVOCATE FOR AND PROVIDE CARING, QUALITY SERVICES TO OLDER ADULTS AND THEIR FAMILIES THAT ENABLE INDEPENDENT LIVING WITH DIGNITY IN THEIR HOME AND COMMUNITY. CHORE SERVICE-HEAVY HOUSEWORK, YARD WORK OR SIDEWALK MAINTENANCE. HOME DELIVERED MEALS LEGAL SERVICES HOMEMAKER SERVICE-LIGHT HOUSEWORK, PREPARING MEALS, MANAGING MONEY, USING THE TELEPHONE OR OTHER HOUSEHOLD ACTIVITIES. RESPITE/ATTENDENT CARE TRANSPORTATION INFORMATION AND REFERRAL CASE MANAGEMENT MEDICAID PRE-ADMISSION SCREENING / WAIVER SERVICES OUTREACH CENTRAL MEALS
