Description
REFER INDIVIDUALS WHO ARE NON-MANAGED CARE AND MEDICAID/MEDICARE DUAL COVERAGE TO A PRIMARY CARE PHYSICIAN, WILL REFER TO A PHYSICIAN FOR PRENATAL CARE AND ASSIST INDIVIDUALS IN ASSESSING BARRIERS TO MEDICAL CARE.
