Description
SUB PROGRAM OF MEDICALLY HANDICAPPED CHILDREN'S PROGRAM. FOR CHILDREN 15 YEARS AND UNDER WHO ARE RECEIVING SSI DISABILITY BENEFITS. THE CASEWORKER, PARENTS AND CHILD DEVELOP AN INDIVIDUAL SERVICE PLAN FOR THE CHILD, BASED ON A NEEDS ASSESSMENT. THE PLAN DESCRIBES THE TYPE AND AMOUNT OF SERVICES TO BE PROVIDED IF THE SERVICES ARE NOT AVAILABLE FROM ANY OTHER AGENCY OR PROGRAM. EXAMPLES ARE: ARCHITECTURAL MODIFICATIONS (RAMPS, WIDER DOORS, ETC.); ATTENDANT CARE (ADDITIONAL FUNDING FOR CHILD CARE); MEALS AND LODGING FOR HOSPITAL STAY; DISABILITY-RELATED TRAINING INCLUDING SIGN LANGUAGE TRAINING FOR PARENTS; RESPITE CARE; SPECIAL EQUIPMENT AND TRANSPORTATION SERVICES. THE PLAN MAY BE REVISED AS NECESSARY.
